Easiest way :
Get TweakUI (Search gOOgle)
Go to "Templates" section
Delete the items.

Create a new and change the extension to the item you want.
for example, if you want to add a new exe file to the menu then create a new
text file and change the extension to exe.
Open TweakUI | Templates | Create and browse to the file.
Click OK
