I'm using the X-Box 360 controller on my windows PC ( XP home, SP2). I use
it to play Final Fantasy XI. If I don't use the controller for a bit, the
guide button which usually just has the "player 1" corner lit up, starts
flashing the whole circle and the controller becomes inoperable until I
unplug it and plug it back in. This will happen whether I am in the game or
not.

Also, I've never been able to get the vibration to work in the game, even
though the game itself supports it and I have it enabled in the games
controller configuration settings.
